1/30/2024 EnchantaVaughn Approved QMHPA greeted Ms. Broadnax upon arrival to the home setting. Upon arrival to the home setting, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA engaged Ms. Broadnax in a discussion surrounding her morning. Ms. Broadnax verbalized to QMHPA that her morning was going good. QMHPA explained to her that this session was going to focus on ways to reduce her depressive and anxiety symptoms. Ms. Broadnax did not present any symptoms of S/I or H/I at the time of the session. QMHPA began the session by inquiring about Ms. Broadnax’s current thoughts, feelings and emotions. QMHPA discussed with Ms. Broadnax the importance of identifying the triggers of her stress, depression and anxiety. QMHPA reinforced to Ms. Broadnax with the importance of taking her psychotropic medication as prescribed needed to stabilize her mental health and mood. QMHPA assisted Ms. Broadnax with conflict resolution skills such as listening and understanding what is being said before reacting/responding as well as walking away from altercations. QMHPA engaged Ms. Broadnax in a skills building activity. QMHPA discussed with Ms. Broadnax that the purpose of this activity was to assist her with the importance of learning to communicate/interact/socialize with other in a positive manner without becoming angry/aggressive. Ms. Broadnax appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Ms. Broadnax successfully identified her mood and demeanor by informing QMHPA that she has been stressed and depressed. Ms. Broadnax shared that not having enough money to purchase the things that she wants and needs causes anxiety. Ms. Broadnax shared that she does not bother people and she gives them warnings when she is becoming angry or upset. Ms. Broadnax admits that she lashes out when people continue to invade her space. Ms. Broadnax was receptive to the skills being taught, but shared that she has difficulty listening and understanding what is being said before reacting/responding. Ms. Broadnax shared that she has never been able to walk away from an altercation or communicate/interact with others in a positive manner particularly when she becomes angry/upset.
